REGISTRATION INSTRUCTIONS
To complete the registration process for the 2012 T-Bird season for football and cheer, please follow the instructions set forth below. Your child’s registration will NOT be complete unless all of the steps have been completed:
1. Please read the Registration Packet. REVISED
2. Click on the document link to review and/or print each document.
For All Participants:

For Cheer Participants Only:

3. Once you have reviewed these documents, PRINT off the Master Acknowledgment form. You will need to complete this form in its entirety by initializing that you have reviewed all of the documents listed above and you and your child are required to sign and date the form where indicated. For Cheer Participants: You will need to indicate on the Master Acknowledgment form where indicated whether your child is authorized to “stunt” as set outlined in the Stunting Permission slip.
4. IF your child has a medical issue that the T-Birds need to be aware of, you will need to print off the Medical Questionnaire and bring it to registration. Remember, it is your sole responsibility to provide the T-Birds with any pertinent medical information that you feel is necessary for the coaches and/or Board of Directors to be aware of. All information will be kept confidential.
5. Register your child “on-line” by clicking HERE. It will re-direct you to the League Toolbox website where you will register your child and pay the necessary fees.
6. Attend one of the “on-site” registrations and bring with you (1) completed Master Acknowledgment form; (2) a certified copy or original of your child’s birth certificate or passport ONLY if your child did not participate in the 2011 season; (3) your child so that a league photo may be taken as well as measurements taken for equipment and, if necessary, (4) the completed Medical Questionnaire; (5) your deposit check.
7. At the “on-site” registration, you will be given the “League Medical Card” referenced in the Registration Packet which will be required to be completed and returned pursuant to the guidelines outlined in the Registration Packet.
